A nonprofit organization and affiliate of the Dickinson Area Community Foundation (DACF) in Iron Mountain, Michigan.
Mission
The Crystal Falls Area Community Foundation (CFACF) encourages philanthropy at all levels, provides donor services and responds to the ever-changing needs and opportunities in the community. The CFACF supports Arts and Culture, Education, Environment, Health and Human Services, and Youth Programs. Donors receive the simplicity and tax advantages of a public charity combined with the personal recognition, involvement and flexibility of a private foundation. Donors also gain access to the highest level of strategic thinking on ways to meet community challenges. We make grants in broad program areas of education, health and human services, arts and culture, environment, community and economic development. In addition to our unrestricted funds, our granting, through field of interest funds, addresses specific needs and issues of our community. To establish a fund reach out to our Executive Director, Tamara Juul at the Dickinson Area Community Foundation.

COMMUNITY IMPACT GRANT
(formerly known as Mini Grants)
The CFACF has four field of interest funds and 34 scholarship funds. The earned income from the field of interest funds go towards grants to area non-profit agencies and organizations within the Crystal Falls area community and scholarships are available to Forest Park High School graduates. Donors may establish new funds or donate to existing funds.
Community Impact Grant (CIG) Fund formerly known as Mini Grants
Our Community Impact Grant Program is available throughout the year for up to $500. The form is located below. The Community Impact Grant is available to 501(c)(3) non-profit organizations, agencies, or organizations that are affiliated with non-taxable organizations i.e. schools, churches, city governments located or servicing people in Dickinson County and surrounding Michigan and Wisconsin areas.

Forest Park Community Projects
This fund supports charitable, educational, cultural, recreational, environmental or social welfare of the people of the Crystal Falls area.
Forest Park Howard Koob Historic
This fund promotes historic preservation of the Harbour House Museum in Crystal Falls until the organization is no longer viably functioned. After that the fund will be used for anything of similar intended purposes (i.e. historical preservation as applied for by a valid functioning area organization or group) in Crystal Falls and the surrounding area.
Forest Park Howard Koob Youth
This fund promotes financial support for the Contemporary Center in Crystal Falls until the organization is no longer viably functioned. After that the fund will be used for anything of similar intended purposes (i.e. youth activities as applied for by a valid functioning organization or group) in Crystal Falls and the surrounding areas.
Forest Park Youth
The Youth Advisory Committee (YAC) made up of Crystal Falls/Forest Park youth distribute the income within this fund.

Scholarship funds are established by donors. Donors identify the criteria for selection. The scholarships listed below are for the Crystal Falls Area Community Foundation. Alina Snell Memorial
Selection Criteria:
One (1) scholarship recipient will be chosen annually from Forest Park High School. Exact award amount is determined annually based upon earnings in Fund.
There are two awards available: one for a male student and one for a female student.
Eligibility:
The student chosen must have been accepted in a 4‐year program at a college or university.
The student's field of study must be in a curriculum leading to a degree: Earth Sciences (Geology, Hydrology, Mineralogy, Meteorology, etc.), Civil Engineering or Medicine.
The student must attach a Personal Statement which includes their comments on how their field of study will lead to a degree in the Earth Sciences (Geology, Hydrology, Mineralogy, Meteorology, etc.), Civil Engineering, or Medicine fields.
The student must be a graduating Forest Park High School Senior.
A transcript of the student's high school records must be supplied demonstrating the student is in the upper 25% academically of his/her graduating class and has obtained a 3.5 Grade Point Average (GPA).
All completed applications must be returned to the Guidance Office by March 15th.
Amasa Gibson
Selection Criteria:
One (1) or more scholarship recipient(s) will be chosen annually from Forest Park High School. Scholarship is renewable up to a 4-year maximum. Exact award amount is determined annually based upon earnings in Fund.
Eligibility:
1. The student chosen must have been accepted in a 2- or 4-year program at a college or university.
2. The student must be a graduating Forest Park High School Senior.
3. First preference shall be given to students who reside in Hematite Township.
4. The student must attach a Personal Statement, which includes their comments on how their field of study relates to this Scholarship.
5. Proof of achievement of a 2.50 GPA or above on a 4-point scale in the preceding year is required to renew the scholarship for the next three (3) years.

Anna & Caroline Schmid Memorial
Selection Criteria:
One (1) or more scholarship recipient(s) will be chosen annually from Forest Park High School. Scholarship is renewable up to a 4-year One (1) or more scholarship recipient(s) will be chosen annually from Forest Park High School. Amount of the award may be up to $4,000 per year for a 4-year maximum. Exact award amount is determined annually based upon earnings in Fund.
Eligibility:
1. The student chosen must have been accepted in a 2- or 4-year program at a college or university.
2. The student must be a graduating Forest Park High School Senior.
3. A transcript of the student’s high school records must be supplied demonstrating the following traits: scholarship and commitment to his or her continuing education. Grade Point Average (GPA) shall not be a consideration for receiving this scholarship.
4. The student shall have demonstrated a strong work ethic throughout their high school years and must provide an acknowledging reference letter from a teacher recognizing their “strong work ethic”, and specific examples of such.
5. The student shall have demonstrated honesty, good citizenship and volunteerism in the community and must provide an acknowledging reference letter from a person who is familiar with the student (not family) recognizing these traits and citing specific examples of such.
6. Students applying for this scholarship will be asked to write an essay on a topic to be determined by the Schmid Scholarship Committee. The writing competition will be held during non-school hours and interested students will be notified of the date and time for the essay competition.
7. The student must attach a Personal Statement, which includes their comments on how their field of study relates to this Scholarship.
8. Financial need of the applicants shall also be taken into consideration.
9. Proof that student remains in good standing at college or university is required to renew the scholarship for the next three (3) years. Students should submit their official college transcripts for the preceding year no later than June 30th of each year to the Dickinson Area Community Foundation at 220 E Hughitt Street, Iron Mountain, MI 49801. Christ United Methodist Church's Gladys Henskey Memorial
Selection Criteria:
One (1) scholarship recipient may be chosen annually. Exact award amount is determined annually based upon earnings in Fund.
Eligibility:
1. Candidates will be considered regardless of age.
2. Candidates will not be limited to high school seniors.
3. Candidates will not be limited to any place of residence.
4. Candidates must have been accepted into a 2 or 4 year program at a college or university.
5. Candidate’s field of study must be in a curriculum leading to a degree in a field of service to people or a community of people.
6. Candidate must attach a Personal Statement, which includes their comments on how their field of study will lead to a degree in a field of service to people or a community of people.
7. Recipient is expected to attend a service of Christ United Methodist Church to receive the scholarship award.
8. All applicants must adhere to Guidance Counselor deadlines. Failure to turn in your application by the due date will result in being ineligible to apply for this scholarship. Guidance Counselors, all completed applications must be returned to Dickinson Area Community Foundation by March 15.
Clark Schwedler Memorial
Selection Criteria:
One (1) or more Scholarship recipient(s) will be chosen annually from Forest Park High School.The recipient(s) shall receive a renewable Scholarship Award in an amount to be determined annually (based upon earned income in the fund).
Eligibility:
1. The scholarship shall be a renewable award with up to 4 equal payments over a 4-year period (or, in special circumstances with the approval of the Dickinson Area Community Foundation Board of Trustees, the 4 equal award payments may be paid out over a 5-year time period.)
2. Applicants must have been accepted at an accredited 2- or 4-year college or university.
3. Applicants must be graduating Forest Park High School Seniors or a past graduate of Forest Park High School who served in the U.S. Armed Forces after graduation and was honorably discharged from service not more than one year prior to the date of application (a “Military Applicant”).
4. Applicants must have a 2 .0 or greater G.P.A. in high school and demonstrated ability to do college-level coursework.
5. Applicants should be students who have demonstrated a noticeable growth in their coursework grades over the years of high school.
6. Applicants must submit a reference letter from a teacher who has known the applicant for a minimum of one (1) year. The reference letter must address the teacher’s personal perception of the student’s capabilities and skills to succeed in their chosen field of study. In addition, the reference letter should address the teacher’s perception of the student’s overall character.
7. In the case of Military Applicant, the Applicant must also submit a reference letter from a commanding officer who has known the applicant for a minimum of one (1) year. The reference letter must address the commanding officer’s personal perception of the Military Applicant’s capabilities and skills to succeed in their chosen field of study. In addition, the reference letter should address the commanding officer’s perception of the Military Applicant’s overall character.
8. Applicants must have demonstrated strong values for themselves and their community through extra-curricular activities and volunteerism throughout their high school years.
9. Applicants must write an essay stating:
♦ How their intended field of study applies to the profession they are pursuing.
♦ How they feel their grades and commitment to their education have grown
♦ How their volunteerism and commitment to community service has made a difference in the community and in their lives. How they have grown as a “leader”, and why they feel that they would be considered a “leader” by their peers.
10. Financial need of the applicant shall be a factor in the selection of students for the Scholarship, but not the sole factor, as outstanding academic achievement and strength of character shall also be considered major factors in student selection.
11. A transcript of the student’s high school records must be supplied.
12. All applicants must adhere to Guidance Counselor deadlines. Failure to turn in your application by the due date will result in being ineligible to apply for this scholarship.

Crystal Falls / Forest Park School Retired Teachers'
Selection Criteria:
One (1) or more scholarship recipient(s) will be chosen annually from Forest Park High School. The recipient(s) will receive a one-time scholarship award. Amount of the award to be determined.
Eligibility:
1. The person chosen must have a sincere desire to attend a college or university.
2. The recipient must be a graduating Forest Park High School Senior.
3. The student must be entering an educational program in an education or health related field of study.
4. A transcript of the student’s high school records must be supplied. Grade Point Average (GPA) shall not be a major consideration for receiving this scholarship.
5. The student shall have demonstrated a good work ethic throughout their high school years and must provide an acknowledging reference letter from a teacher recognizing their “good work ethic”, and specific examples of such.
6. The student must have one character reference from a teacher and one from a person who is familiar with the student (not family).
7. The student must provide a three (3) paragraph personal statement outlining reasons for pursuing either the field of education or health.
8. All completed applications must be returned to the Dickinson Area Community Foundation Office by March 15th. Failure to turn in your application by the due date will result in being ineligible to apply for this scholarship.

Dahms Family
Selection Criteria:
One (1) or more scholarship recipient(s) may be chosen annually from Forest Park High School, depending upon funds available. Exact award amount is determined annually based upon earnings in Fund.
Eligibility:
1. The student chosen must have been accepted in a post high school training program of at least one school year in length. Preference will be given to students pursuing 1 or 2 year program in the vocational/trade/technical areas.
2. The student must attend the training program during the year immediately following his/her high school graduation, beginning with the first regular semester or term.
3. Scholarship recipients must be a graduate of Forest Park High School or a resident of Eastern IronCounty which consists of Alpha, Crystal Falls and Amasa.
4. The intent of the Dahm’s Family Scholarship is to provide financial aid to applicants who otherwise might not receive any other scholarships due to GPA or class ranking. The Scholarship Selection Committee shall strive to meet the Donors’ wishes before granting the scholarship in any other manner.
5. Payment of one half of the scholarship is made to the recipient in mid-August or as soon as the recipient provides verification of registration. The remainder of the annual scholarship is paid at the beginning of the second semester once an official transcript verifying of successful completion of the first semester is provided to the Dickinson Area Community Foundation, 220A E Hughitt Street, Iron Mountain, MI 49801 (Phone: 906-774-3131).
6. All applicants must adhere to Guidance Counselor deadlines. Failure to turn in your application by the due date will result in being ineligible to apply for this scholarship.

Donald & Jean Boulanger
Selection Criteria:
The family will determine the amount of the scholarship and how many to be awarded once they receive information on what is available each year. Exact award amount is determined annually based upon earnings in the Fund.
Eligibility:
1. Available to graduating Forest Park seniors and non-traditional students from the Crystal Falls areas who have been out of school and are returning to school.
2. Students must show proof they are enrolled in their program of choice before the scholarship is awarded.
3. Must be pursuing a certification less than a bachelor degree in a Vocational or Technical school in a program but not limited in the following; building trades, welding, auto mechanics, machine tool, electrical, health care, dietary or agriculture.
4. Students must have demonstrated a degree of work ethics in high school and the community.
5. GPA may be used as a determining factor but more important the student should have a firm foundation in the area related to his/her training.
6. Must submit a one-page essay 12 point font, double-spaced not more than one page: on how they have demonstrated their work ethic in school and the community and what made you choose the vocational or trade program you are entering. Also, how this scholarship will help you meet that goal.
7. All applicants must adhere to Guidance Counselor deadlines. Failure to turn in your application by the due date will result in being ineligible to apply for this scholarship.

Selection Criteria:
One scholarship is awarded to a graduating senior. Exact award amount is determined annually based upon earnings in Fund.
Eligibility:
1. The student chosen must have been accepted in a program at a college, university, or technical school.
2. The student must be a graduating Forest Park High School Senior.
3. The student must be pursuing a degree in a business or commercial field.
4. The student must attach a Personal Statement, which includes their comments on how their field of study relates to the Business or Commercial fields.
5. All applicants must adhere to Guidance Counselor deadlines. Failure to turn in your application by the due date will result in being ineligible to apply for this scholarship.

Eugene & Kathryn Sedberry
Selection Criteria:
One (1) scholarship recipient will be chosen annually from Forest Park High School. Exact award amount is determined annually based upon earnings in Fund.
Eligibility:
1. The student chosen must have been accepted at an accredited college or university.
2. The student must be a graduating Forest Park High School Senior.
3. The student must attach a Personal Statement, which includes their comments on how their field of study relates to this Scholarship.
4. Three one-time scholarships will be awarded.
5. All applicants must adhere to Guidance Counselor deadlines. Failure to turn in your application by the due date will result in being ineligible to apply for this scholarship.

Father Gondek
Selection Criteria:
One Scholarship recipient will be chosen annually from Forest Park High School. Exact award amount is determined annually based upon earnings in Fund.
Eligibility:
1. The student must be a graduating Forest Park High School Senior.
2. Selection will be based on the priority considerations listed below:
a. First consideration is to be given to a boy entering a seminary to study for the Priesthood.
b. In the event the first priority does not materialize, the scholarship would then be granted to a girl entering a religious vocation.
c. Should the first two considerations fall short of a recipient, the scholarship is then to be awarded to a qualified boy or girl pursuing a career in the field of music whose plans would include studying toward a degree.
d. Should none of the above materialize, consideration may be given to a past alumnus who meets the criteria for “a” or “b” in that order. Item “c” is excluded from this consideration.
3. A transcript of the student’s high school records must be supplied demonstrating the following traits: scholarship and a potential for success in his or her continuing education.
4. The student chosen shall have demonstrated the following characteristics throughout high school: Christian maturity, character, leadership potential, and personality, and must provide an acknowledging reference letter from a teacher or from a person who is familiar with the student (not family) recognizing these traits and citing specific examples of such.
5. The student must attach a Personal Statement, which includes their comments on how their field of study relates to this Scholarship.
6. Financial need of the applicants shall also be taken into consideration.
7. All applicants must adhere to Guidance Counselor deadlines. Failure to turn in your application by the due date will result in being ineligible to apply for this scholarship

Selection Criteria:
One (1) scholarship recipient will be chosen annually from Forest Park High School. Exact award amount is determined annually based upon earnings in Fund.
Eligibility:
1. The student chosen must have been accepted at an accredited college or university.
2. The student must be a graduating Forest Park High School Senior.
3. A transcript of the student’s high school records must be supplied demonstrating an overallGrade Point Average (GPA) of 2.0.
4. All applicants must adhere to Guidance Counselor deadlines. Failure to turn in your application by the due date will result in being ineligible to apply for this scholarship.

Selection Criteria:
One (1) scholarship recipient may be chosen annually from Forest Park High School. Exact award amount is determined annually based upon donations received to the Fund (1/2 of annual donations received in a given year are awarded).
Eligibility:
1. The student chosen must have been accepted in a 4-year program at an accredited college or university.
2. The student must be a graduating Forest Park High School Senior.
3. The student must be pursuing an education in business.
4. The student must attach a Personal Statement, which includes their comments on how their field of study relates to the Business field.
5. A transcript of the student’s high school records must be supplied demonstrating an overallGrade Point Average (GPA) of 2.75.
6. All applicants must adhere to Guidance Counselor deadlines. Failure to turn in your application by the due date will result in being ineligible to apply for this scholarship

Forest Park Humanities
Selection Criteria:
One (1) scholarship recipient will be chosen annually from Forest Park High School. Exact award amount is determined annually based upon earnings in Fund.
Eligibility:
1. The student chosen must have been accepted at a college or university.
2. The student must be a graduating Forest Park High School Senior.
3. The student must be studying in the field of humanities.
4. The student must attach a Personal Statement, which includes their comments on how their field of study relates to the Humanities field.
5. All applicants must adhere to Guidance Counselor deadlines. Failure to turn in your application by the due date will result in being ineligible to apply for this scholarship.

Forest Park Science
Selection Criteria:
One (1) scholarship recipient will be chosen annually from Forest Park High School. Exact award amount is determined annually based upon earnings in Fund.
Eligibility:
1. The student chosen must have been accepted at a college or university.
2. The student must be a graduating Forest Park High School Senior.
3. The student must be studying in the field of science.
4. The student must attach a Personal Statement, which includes their comments on how their field of study relates to the Science field.
5. A transcript of the student’s high school records must be supplied demonstrating an overallGrade Point Average (GPA) of 2.0.
6. All applicants must adhere to Guidance Counselor deadlines. Failure to turn in your application by the due date will result in being ineligible to apply for this scholarship.

Forest Park Vocational/Technical
Selection Criteria:
One (1) scholarship recipient will be chosen annually from Forest Park High School. Exact award amount is determined annually based upon earnings in Fund.
Eligibility:
1. The student chosen must have been accepted at a technical/vocational school.
2. The student must be a graduating Forest Park High School Senior.
3. The student must attach a Personal Statement, which includes their comments on how their field of study relates to this Scholarship.
4. All applicants must adhere to Guidance Counselor deadlines. Failure to turn in your application by the due date will result in being ineligible to apply for this scholarship.

Selection Criteria:
One (1) scholarship recipient will be chosen annually from Forest Park High School. Exact award amount is determined annually based upon earnings in Fund.

1. The student chosen must have been accepted at an accredited college or university.
2. The student must be a graduating Forest Park High School Senior.
3. The student must be pursuing a career in the Education field.
4. The student must attach a Personal Statement, which includes their comments on how their field of study relates to the Education field.
5. All applicants must adhere to Guidance Counselor deadlines. Failure to turn in your application by the due date will result in being ineligible to apply for this scholarship.

Gus & Vi Eckola
Selection Criteria:
One (1) or more scholarship recipient(s) will be chosen annually from Forest Park High School. The recipient(s) will receive a one-time scholarship award. Amount of the award to be determined annually.
Eligibility:
1. The recipient shall be a graduate of Forest Park High School; and shall be a student entering in a vocational/technical or college/university educational program in a music field of study (instrumental, vocal, or educational). If no applicants fulfill the music field of study, second consideration shall be a student entering a vocational/technical or college/university educational program in an education field of study.
2. The recipient shall have a Grade Point Average (GPA) of no less than 2.0.
3. A three (3) paragraph personal statement must be attached stating how your field of study relates to either the music or education field you are entering into.
4. One scholarship will be awarded.
5. All applicants must adhere to Guidance Counselor deadlines. Failure to turn in your application by the due date will result in being ineligible to apply for this scholarship.

Howard F Koob Memorial
Selection Criteria:
Scholarship recipients may be chosen annually from Forest Park High School. Scholarship(s) will be awarded to student(s) pursuing a 1-2 year technical/vocational degree, and student(s) pursuing a 4-year degree in any field. Exact award amount and number of awards is determined annually based upon earnings in Fund.
Eligibility:
1. The students chosen must have been accepted in a 1–2-year program at a technical/vocational school, or to a 4-year program at a college or university.
2. The recipients must enroll and attend the program during the school year immediately following his/her high school graduation, beginning with the first regular Fall semester or term.
3. The student must be a graduating Forest Park High School Senior.
4. A transcript of the student’s high school records must be supplied demonstrating the student’s scholastic ability, good possibility of continued academic success and seriousness of purpose.
5. Applicants shall be judged on citizenship and participation in extra-curricular activities.
6. The student must attach a Personal Statement, which includes their comments on how their field of study relates to this Scholarship.
7. Applicants must be able to show that financial need exists.
8. All completed applications must be returned to the Guidance Office by March 15th.

Iron County Timberman's and Eddie Meyers Scholarship
Four or more scholarship recipients will be chosen annually from Forest Park High School and West Iron County High School. The recipients will receive a one-time or a renewable scholarship award. The award amount and the number of scholarships awarded will be determined by the Dickinson Area Community Foundation (DACF) Board of Trustees and the Iron County Timbermen and Eddie Meyers Scholarship Committee.
Eligibility:
The scholarship is available to graduating seniors of Forest Park High School and West Iron County High School. In addition to seniors, any exceptionally gifted student who will be proceeding directly to a college/university in the subsequent academic year without graduating from high school, such as a home-schooled student, is eligible to apply.
Applicants must have been residents of Iron County for the year prior to applying.
Applicants must have been accepted at an accredited two or four-year college or university; or entering the “trades” field through an apprenticeship, certificate, or associate degree program; or entering a branch of the armed forces.
Preference will be given to applicants pursuing a forestry related degree or certificate or applicants with a family history in the field of forestry.
A transcript of the student’s high school records must be supplied.
Applicants must submit a personal statement on why they have chosen their field of study and how forestry has impacted their life.
The scholarship committee’s estimation of an applicant’s potential for success in the intended program will be a major factor in determining the awards. Scholarship, citizenship, extra-curricular activities and financial need of the applicant will also be considered, as well as seriousness of purpose.
Applications must be completed and on file in the counselor’s office by March 15th.
The recipient must provide proof of acceptance at their chosen institute of higher learning or branch of military before awards will be dispensed.
The scholarship may be a one-time or a renewable award. Renewable awards will consist of equal annual payments. For a renewable scholarship, the recipient must maintain a full-time course schedule and a GPA of 2.0 or higher. The recipient must submit their official college transcript to the DACF prior to award disbursement to confirm eligibility.
In the event the Scholarship Selection Committee does not feel applicants for a particular year warrant the Scholarship, then no award shall be given out and eligible funds shall remain in the Scholarship Fund account.
A scholarship award may be revoked by the DACF Board of Trustees because of:
a. Criminal or anti-social conduct of recipient,
b. Filing false information on application,
c. For such other good cause as the Board may, in its sole discretion, determine.
Revocation shall be by the action of a majority of the members of the DACF Board of Trustees, and upon such revocation, any and all funds still controlled by the Board of Trustees, shall be withheld and disposed of at the discretion of the Board.

Selection Criteria:
One or more scholarship recipients may be chosen annually from Forest Park High School to receive a one-time scholarship award. The award amounts are determined annually based upon the earned income of the Fund over the preceding year.
Eligibility:
1. The recipient must be a graduating Forest Park High School senior.
2. The scholarship is for a one-year period only and may not be renewed.
3. The recipient must be entering a degree granting program at a vocational/technical school or a 2- or 4-year post-secondary institution of higher learning.
4. Applicants must have at least a 2.0 GPA.
5. The individual financial need of the applicant for scholarship assistance will be a consideration of the scholarship committee when applicants have met all other criteria fort he scholarship.

1. Available to graduating Forest Park seniors with a 2.5-3.0 GPA (preferred).
2. Students must show proof they are enrolled in a Postsecondary Education (including vocational and trade schools) before the scholarship is awarded.
3. Students must be pursuing a Postsecondary Education in a vocational and/or trade school.

1. The student must have been accepted in a public institution of higher learning in the State of Michigan.
2. The student must be a graduating Forest Park High School Senior.
3. The student shall have demonstrated service to the community and must provide an acknowledging reference letter from a person who is familiar with the student (not family) recognizing the student’s commitment to the community and citing specific examples of the service(s) the student has provided to the community.
4. The student shall have demonstrated honesty, good citizenship and volunteerism in the community and must provide an acknowledging reference letter from a person who is familiar with the student (not family) recognizing these traits and citing specific examples of such.
5. The student must attach a brief statement (3-4 paragraphs) addressing the following:
a. How have the services you provided to the community impacted the community?
b. How have the services you have provided to the community impacted you?
c. What suggestion do you have for involving young people in community service in the future?
6. The student must attach a Personal Statement, which includes their comments on how their field of study relates to this Scholarship.

One or more scholarship recipient(s) will be chosen annually from Forest Park High School. The recipient(s) will receive a one-time scholarship award. The award amount and the number of scholarships awarded will be determined annually by the Dickinson Area Community Foundation (DACF) Board of Trustees and the Scholarship Selection Committee.
Eligibility:
1. The scholarship is available to graduating seniors of Forest Park High School. In addition to seniors, any exceptionally gifted student who will be proceeding directly to a college/university in the subsequent academic year without graduating from high school is eligible to apply.
2. The scholarship is for a one-year period only and may not be renewed.
3. Applicants must have been accepted at an accredited vocational/technical trade school or two- or four-year college or university.
4. Applicants must have a 3.0 or greater G.P.A. in high school and have demonstrated the ability to do college-level coursework.
5. Applicants must have participated in an athletic sport during high school.
6. Preference will be given to students who meet most or all of the following criteria:
a. Students who are entering a medical field.
b. Students who have been involved in a 4-H club.
c. Students who were employed while in school.
d. Students who will be attending Michigan State University.
e. Students who have demonstrated honesty, integrity, kindness, compassion, teamwork, and love of animals.
7. Applicants must submit an essay (approximately 250 words) describing how they have demonstrated the above characteristics.
8. Applicants must submit a reference letter from a teacher or a member of the community who has known the applicant for a minimum of one (1) year. The reference letter must address the writer’s personal perception of the student’s overall character. In addition, the reference letter should address the writer’s perception of the student’s capabilities and skills to succeed in their chosen field of study.

One (1) or more Scholarship recipient(s) will be chosen annually from Forest Park High School. The recipient(s) shall receive a one-time Scholarship Award in an amount to be determined annually (based upon earned income of the fund).
Eligibility:
1. Applicants must have a sincere desire to continue her education in an accredited vocational/technical school or college/university.
2. Scholarship recipients must be a female graduate of Forest Park High School.
3. Applicants must show they demonstrated service to the community.
4. Applicants must have demonstrated leadership potential.
5. Applicants must submit three non-relative references with their contact information. One reference must be a peer and one must be non-academic. Your references should be able to show the committee your community service and leadership involvement.
6. Applicants must submit a five-to-ten-minute video (MP4 file). The video should tell the committee about your community service activities and demonstrate your leadership skills. In addition, it should include how you have positively impacted your peers and how you have been inspired by leaders in your life.
7. A transcript of the student’s high school records must be supplied.

6. Verification of your fall schedule with declared major will have to be provided in order to receive the first payment of the scholarship.
